    Can A Parent Sleep With The Child In Bed

    The quality of your childs test is very important. Parental co-sleeping with your child can affect the quality of the sleep study. Your help prior to and during the study is very critical in obtaining the best results from your childs sleep study but we ask that you refrain from sleeping with your child during the sleep study.

    When Should I Refer Patient To Sleep Expert

    • When a suspected sleep disorder is resistant or needs more time or attention than a simple clinic visit allows.
    • When a child with developmental delays or special needs requires a sleep study or when extreme anxiety even to a simple PSG for OSA is predicted.
    • When CPAP/Bi-level PAP is required. Children more than adults need careful mask-fitting, habituation and education.
    • When hypersomnia is a significant complaint. A consult will help assess the need for a next-day MSLT, actigraphy study, or analysis of sleep logs for confounders.
    • When RLS is suspected. A consult can evaluate secondary causes and comorbid periodic limb movement disorder .
    • When parasomnias are severe, frequent or dangerous, or when nocturnal motor activity is suspicious for seizures. An expanded sleep study with full EEG or extra-limb leads may be needed.

    Our Approach To Sleep Problems

    Our specialists use state-of-the-art computerized sleep study equipment to aid in the diagnosis of your childs sleep disorder. The pediatric sleep laboratory is comfortable, child-friendly, and staffed by seasoned sleep technicians who are expert at tabulating information about your childs sleepcharacteristics. You will be able to stay with your child throughout the study.

    Our team reviews the results of your child’s study with the aim of designing a treatment plan to improve the quality of your childs sleep. When necessary, we use a multidisciplinary approach to address your childs condition, working with pediatric ENT physicians, obesity experts, bariatric surgeons, developmental specialists, craniofacial surgeons, neurosurgeons, neuromuscular physicians, and gastroenterologists.

    We also have two nurse practitioner/educators on hand to help you and your child accommodate to CPAP/BPAP titration for complex sleep disorders. To help with other issues, we have a social worker on staff as well as access to behavioral therapists.

    Your First Sleep Clinic Visit

    During a your first visit to our sleep clinics, you will meet a team of providers, including physician trainees who will be our nations future sleep specialists. An initial assessment with a detailed history and clinical exam by a sleep provider will be the first step.

    In this evaluation, further studies may be suggested, including sleep studies and possible laboratory and radiological testing. A complete diagnostic approach and therapeutic plan will be provided and communicated to the referring provider.

    What Is A Sleep Study

    Sleep study refers to group of nighttime studies most common of which is polysomnogram . PSG is objective way to evaluate for various sleep disorder. During a PSG, a patients brainwaves , heart rate, breathing and limb movements are monitored. PSGs are typically one night study attended by a sleep lab technician and take place at a sleep lab, which may be located in a medical facility or a hotel. PSG is most often used to assess sleep related breathing disorders , to titrate CPAP or Bi-level PAP, or to assess periodic limb movements.     What To Expect During A Sleep Study

    If you are coming to our laboratory for a sleep study , you and your child are probably wondering what to expect.

    In this step-by-step video walkthrough of a sleep study, your family can follow a patient as he completes a night in the Sleep Lab.

    Most sleep studies are completed at night. They typically start between 7 pm and finish between 5:30 and 6 am. Physicians are not present during the study. During a sleep study, the sleep technologist will place sensors on your childs body to record a number of different body functions. These sensors are not invasive and do not break the skin. They are used to monitor:

    • Brainwaves
    • Breathing patterns, carbon dioxide and oxygen levels
    • Heart rate and rhythm
    • Leg movements

    The sleep technologist will also record your child, using both audio and video, to watch how your child sleeping pattern and assess and hear your childs breathing and snoring during sleep.

    Dealing With Sleep Disorders

    We cater for infants and children aged 0 to 16 years and up to 18 years if they are still schooling. The Clinic deals with the entire spectrum of sleep disorders such as sleep-breathing problems, narcolepsy, sleep-walking, night-terrors, behavioural sleep problems and so on. Patients referred to our practice for a sleep study will first attend a consultation with one of our physicians to assess your child. If a sleep study is required, it will determine how soon this should be performed and an admission arranged to either Sydney Children’s Hospital or St Luke’s Private Hospital to have the sleep study performed.

    You will be required to bring a referral letter to your appointment – without it you will not be able to claim a rebate from Medicare.

    Management And Monitoring Of Positive Airway Pressure

    An effective therapy to treat obstructive sleep apneas and sleep-disordered breathing in children and adolescents is with non-invasive positive airway pressure . PAP therapy involves the delivery of room air by way of a bedside machine that delivers a stream of air to the childs airway using an oral or nasal mask. This stream of air prevents airway collapse.

    What Is The Role Of Sleep Behavioral/psychology Provider

    What Are The Indications For Sleep Study

    Indications for sleep study include snoring and sleep related breathing disorders, and excessive daytime sleepiness. PSGs are particularly useful in assessing for sleep related breathing disorders in the child with obesity, developmental or neuromuscular disorder, craniofacial abnormality, school failure or behavioral problems. The overall prevalence of sleep apnea is 1 to 3 percent among otherwise normal children, 15-63% in obese children, 20 to 30 percent in children with ADHD, and 50 to 75 percent in Down syndrome.

    Types Of Sleep Disorders

    The most common are:

    Sleep apnea: Up to 4% of children, typically ages 2 to 8, have sleep apnea. The two types are:

    • Obstructive sleep apnea: The most common type occurs when soft tissue at the back of the throat blocks the upper airway. This cuts off oxygen for a few seconds until the child wakes up and breathes normally.
    • Central sleep apnea: This happens when the brain stops signaling a child to breathe, sometimes for a minute or longer. Its usually linked to other conditions that involve the central nervous system .

    Insomnia: Your child has trouble falling asleep or staying asleep, or wakes too early. Causes can be poor sleeping habits, stress or anxiety. In the U.S., 45% of adolescents dont get enough sleep, and some may suffer from insomnia, according to National Sleep Foundation.

    Delayed phase sleep syndrome: This chronic disorder, common among teens, occurs when they go to bed two or more hours later than normal. This makes it hard to wake up the next morning.

    Excessive sleepiness: This is unexplained sleepiness, with no other sleep disorder or related medical issue.

    Periodic limb movement disorder: An urge to move or kick the legs repeatedly wakes the child. Scientists have linked this disorder to iron deficiency.

    Parasomnia: This set of conditions is marked by abnormal movements during sleep. Doctors sometimes call it things that go bump in the night. Common types include:
